About Community Paramedicine

Community Paramedicine is an extension of primary health care that is an accessible, reliable, and safe approach to help meet community health needs. Each of our Community Paramedics have additional training beyond the traditional 911 emergency response. The goal of the Community Paramedicine program is to prevent unnecessary emergency room visits and hospital admissions by providing non-urgent medical care within your home. We offer a range of services including chronic disease management, remote patient monitoring, health education, assessment, treatment, and referrals to other agencies.

There is no cost for this service, and home visits and telephone consults are scheduled based on client need. All Community Paramedicine programs operate 7 days a week, with some operating 24/7.

The Community Paramedic Program is not a substitute for emergency care, family physician appointments, or walk in clinic services. We work alongside your existing services to provide additional support.

Clients must reside in the City of Greater Sudbury

There is no need to select a specific Community Paramedicine program when sending a referral, our triage team will determine the best fit based on the needs described in the referral form. To ensure referrals are processed in a timely manner, please complete all sections.
